Are you looking at realligning it, or does the hinges need new bushings and pins? Have you replaced the bushings and pins? After that, you can add shims between the hinges and the doors or at the cab if the hinges aren't welded or glued to the cab. Other than that you will likely have to move the fender instead of the door. But that is only if the door is hung right. You can check this by looking at the gaps between the door and the cab near the door handle. If those gaps at the rear of the door are good and the gap at the fender is not, then move the fender and reallign the fender.
That is what I would do, but it all depends on what needs to move and where it needs to move to. But start with the hing pins and bushings, they are the first thing that wears out.
